 I always thought that the 2 speed pto box was a completely seperate thing to the main gearbox. I could be wrong though, it's certainly not unusual!

Why do you need the pto shaft to be central? I thought the two uj's on each end of the shaft flexed to allow for an offset? They certainly do when you raise and lower the linkage anyway, so I can't see the other angle being any different?  :-\
